Warfarin and Direct-Acting Anticoagulants: A Review for Pharmacy Technicians
Review of the medications used for anticoagulation
including warfarin, injectable anticoagulants and
the direct acting anticoagulants (DOACs). We will
discuss how they differ in terms of mechanism of
action, indications and interactions. We will also
learn about how they are monitored and how we manage
bleeding events with the different agents. From a
technician standpoint, we will review the relevant
information that should be escalated to a pharmacist
or medical provider when interacting with patients who
are using anticoagulant therapy.
Fall Seminar attendees will be able to:
Understand the situations that could necessitate dose changes or increased monitoring in patients taking warfarin.
Identify the oral and injectable anticoagulant agents used by patients in the outpatient setting.
Describe “red flags” when dealing with patients taking anticoagulants that would lead you to escalate an interaction to the pharmacist or medical provider.

Unpacking the INR Blood Test- A Discussion for Pharmacy Technicians
This is a discussion of the INR blood test, including its history,
clinical uses and role in monitoring warfarin. We will discuss uses
of the INR test, how the test is performed and caveats to interpreting
the value.

Discuss INR normal and abnormal ranges.
Describe risk associated with either a low INR or a high INR.
